Discharge
DISCHARGE PROCESS
The medical team will review the condition and treatment progress to determine if the patient is ready for discharge.
Once the patient meets the discharge criteria, a discharge plan will be created, which includes instructions for follow-up care, medications, and any lifestyle changes needed.
A discharge summary will be prepared, detailing the patient’s hospital stay, diagnosis, treatments received, and instructions for follow-up care.
The medical team will provide prescriptions for any medications you need to take after leaving the hospital, along with instructions on how and when to take them. Patient can collect the medications from the pharmacy counter.
Schedule any follow-up appointments as per the advice of your doctor.
